Registering Foundling

ABOUT THE SERVICE:

A foundling is a deserted or abandoned infant/child who relatives are unknown or a child committed in an orphanage or similar institutions and whose facts of birth and percentage are unknown.

 

The Civil Registry Law provides that the registration of foundling in the municipality where the child was found shall be made by the finder/charitable institutions within thirty (30) days from the date of finding/commitment of the child.

 

CLIENT GROUPS:

Finder or charitable institutions

 

REQUIREMENTS:

  • Certificate of Founding
  • Affidavit of the Finder
  • Certification of the Brgy. Captain or police authority regarding the report

 

SERVICE SCHEDULE:

Monday to Friday 8:00 AM to 12:00 noon, 1:00 PM to 5:00 PM

 

FEES/CHARGES:

On time registration – P20.00

More than 1 month but less than one year – P50.00

One year but less than 10 years – P75.00

10 years and above – P100.00

Death Certification

ABOUT THE SERVICE: It shall be the responsibility of the nearest relative or spouse who has knowledge of the death to report the same within forty eight (48) hours if the deceased died without medical assistance. The Health Officer shall examine the deceased and shall certify as to the cause of death and direct the registration of the death certificate to the Municipal Civil Registrar within the reglementary period of thirty (30) days.

Marriage Licensing

ABOUT THE SERVICE: In ordinary marriage, the time for submission of the Certificate of Marriage is within fifteen (15) days following the solemnization of marriage while in marriage exempt from license requirements, the prescribed period is thirty (30) days, at the place where the marriage was solemnized.
